Registration Information
Before your child can be officially enrolled in Butterflies and Bumblebees Daycare you must complete and provide the following documents:
*spaces will not be held by verbal contract, registration fee and deposit must be paid in order for a spot to be held for you and your child.
- Signed Parent Contract and Rate Agreement
- Completed Child Profile & Copy of Immunization Record
- Signed Consent Forms (those that are applicable)
- Registration Fee must be paid ($25)
- $25 Supply Account Fee - this is an account that we will hold in your name at the daycare that will be used in cases where you child has run out of important supplies (such as diapers, creams, sunscreen, etc.).
